Request for Permission to Publish

If you would like to use an image from the museum archive you may search the online database, a collaborative project in partnership between the museum and the Los Gatos Library. You may also request a specific image by submitting an email inquiry. Prior to use, fill out the online form below, we’ll get back to you within 7-10 business days to confirm receipt and clarify any questions. After the form has been processed, please select the payment option that best fits your request and we’ll send you written approval and the image file. PERMISSION TERMS:

Permission to publish granted by New Museum Los Gatos (NUMU) does not constitute copyright clearance or right of publicity release; materials owned by NUMU which may be subject to copyright will also require written permission from the current copyright owner(s), or their heirs or assigns. - Photographs or other materials may not be altered. Any additions or other changes including cropping or use in text or graphic overlay must be preceded by written permission from NUMU. - Permission to use materials from the collections is granted on a one-time basis only, and for the purpose stated on this form only. Publication (printed or electronic), copying or displaying of photographs, or other material, for purposes not stated on this form is prohibited. - Permission for use is granted only for the materials listed on this form. Use of these materials for any other purpose, or for the same purpose more than once must be preceded by signing a permission form each time the materials are used. Permissions are non-transferrable. - NUMU holds the right to withdraw permission to use materials from its collections if these terms are not followed by the user. Please note: A credit line: Courtesy of New Museum Los Gatos, or similar variation must be used on all items or materials NUMU grants permission to use. If exhibited, the credit line should be in close proximity to the image or item displayed. We request a copy or tear sheet (printed or electronic) of your publication for our records.
